丹参多酚酸盐治疗早期慢性肾衰竭及对血浆ET-1和CGRP水平的影响 被引量:28

摘要 目的探讨丹参多酚酸盐在治疗早期慢性肾衰竭中的作用及机制。方法 54例患者随机分为2组,常规治疗组27例(简称常规组);常规+丹参多酚酸盐治疗组27例(简称研究组)。观察丹参多酚酸盐治疗前后肾功能、尿蛋白及血浆ET-1和CGRP水平的变化。结果 CRF患者血浆ET-1水平高于对照组,而血浆CGRP水平偏低;两者在治疗后均显著改善,但以研究组改善更为显著,与常规组治疗后比较,血浆ET-1水平降低(P<0.05),而血浆CGRP水平增高(P<0.01)。CRF患者血浆ET-1水平与血清Scr、尿蛋白呈正相关;而血浆CGRP水平与后两者均呈负相关;血浆ET-1水平与CGRP呈负相关。丹参多酚酸盐治疗后,血浆ET-1水平与Scr、尿蛋白及血浆CGRP之间的相关关系和治疗前一致。结论丹参多酚酸盐能降低CRF患者血浆ET-1水平,增加血浆CGRP水平,减少尿蛋白,改善肾功能,故值得临床应用。 Objective To study the effect of salvianolate injection in treating patients with early stage of chronic renal failure.Methods Patients with CRF were randomly divided into two groups.The routine treatment group(RT group)was treated with basic therapy,such as diet therapy,compound α-keto acid、erythropoietin and antihypertensive,et al.The study group(ST group)was treated with rountine medicine and salvianolate,et al.The plasma ET-1 and CGRP levels were detected before and after treatment and also control group.Results The plasma levels of ET-1 of CRF patients were higher than those in normal control group before treatment(P0.01),and The plasma CGRP levels were lower(P0.01).The changes of ET-1,CGRP before and after treatment were significant,the improvement of ET-1,CGRP in the ST group was better than that in the RT group(P0.05,P0.01,respectively).Plasma ET-1 were significantly and positively correlated with Scr,urinary protein(r=0.5700,P0.01;r=0.4840,P0.05,respectively),and the plasma levels of CGRP were negatively correlated with Scr、urinary protein(r=-0.5070,P0.01;r=-0.4210,P0.05,respectively).Plasma ET-1 negatively correlated with plasma CGRP(r=-0.4210,P0.05).So was their correlation ship after the salvianolate treatment.Conclusion The result reveals that salvianolate has obvious effect on patients with CRF by improving the unblance of ET-1 and CGRP,reducing urinary protein and improving renal function.So it was worth of application.
