高血压特急性脑出血78例临床分析

Clinical analysis of 78 patients with acute hypertensive cerebral hemorrhage

目的探讨高血压特急性脑出血临床处理方法及应用效果。方法回顾分析我院2002年4月～2010年5月所收治高血压特急性脑出血患者78例的临床资料,将78例患者分为直接开颅组与术前快速锥颅组,对两组的治疗结果进行统计学分析。结果直接开颅组患者的GOS结果分级差于快速锥颅组,χ2=4.38,P<0.05,差异有统计学意义,而两组患者远期治疗结果 ADL无明显差异,χ2=3.36,P>0.05,差异无统计学意义。结论快速有效的排除部分脑内血肿,缓解血肿对脑干的压迫,降低颅内压,可以明显提高患者的生存率。

Objective To explore the treatment and efficacy of acute hypertensive cerebral hemorrhage.Methods The clinical records of 78 cases with acute hypertensive cerebral hemorrhage from April,2002 to May,2010 were analyzed.The 78 cases were divided into two groups：preoperative rapid Awl cranium drainage and direct craniotomy so as to analyze the treatment effect of the two groups statistically.Results The Glasgow outcome scale（GOS） of the patients treated with direct craniotomy was worse than that with preoperative rapid Awl cranium drainage（P〈0.05）.But for the long-term effects,activities of daily living（ADL） in two groups had no difference.Conclusion Cleaning up partial incranial hematoma fast and effectively,releasing pressure from hematoma to brain stem,and decreasing ICP can improve the survival rate obviously.
