舒张性心力衰竭患者血清N末端-脑钠肽前体水平检测分析 被引量:4

Determination of N-terminal pro-brain natriuretic peptide(NT-proBNP)level in patients with diastolic heart failure

摘要 目的探讨舒张性心力衰竭(DHF)患者血清N末端-脑钠肽前体(NT-proBNP)水平的变化及临床意义。方法选择63例失代偿性DHF患者为DHF组,40例无器质性心脏病者为对照组。所有受试者均采用NYHA分级法分级诊断心功能,电化学发光双抗体夹心免疫法检测血清NT-proBNP水平,心脏彩色多普勒超声测定左心室舒张功能。对比两组的血清NT-proBNP水平,分析DHF患者血清NT-proBNP水平与NYHA心功能级别和左心室舒张功能障碍程度的关系。结果 DHF组患者血清NT-proBNP水平显著高于对照组(P<0.05),并随NYHA心功能级别增高和舒张功能障碍程度加重而升高(P<0.05)。结论血清NT-proBNP水平能在一定程度上反映失代偿性DHF患者的心功能状态,有助于DHF的诊断和病情评估。 Objective To investigate the serum N-terminal pro-brain natriuretic peptide(NT-proBNP)level in patients with diastolic heart failure(DHF)and its clinical significance.Methods 63 patients with decompensated DHF were selected as DHF group,and other 40 patients without organic heart disease were selected as the control group.The heart functions of all subjects were evaluated based on the New York Heart Association(NYHA)grading.Serum NT-proBNP level was assayed by electrochemiluminescence double antibody sandwich immunoassay.Left ventricular diastolic function was detected by color Doppler ultrasound.The differences of NT-proBNP level between DHF group and the control group were compared,and the correlations among NT-proBNP level,NYHA cardiac functional class and the severity of left ventricular diastolic dysfunction were analyzed.Results NT-proBNP level in DHF group was significantly higher than that in the control group(P〈0.05).Furthermore,it showed an increased tendency along with the increase of cardiac functional grading and the aggravation of cardiac diastolic dysfunction(P〈0.05).Conclusion Serum NT-proBNP level can demonstrate the cardiac functional grading in patients with decompensated DHF.Therefore,it is helpful for the diagnosis and prognosis of DHF.
