根据体重对比剂注射方案在64层螺旋CT冠状动脉成像中的应用

Application of contrast media injection protocol to 64～slice spiral CT coronary angiography

目的:研究根据体重对比剂注射方案在64层螺旋CT冠状动脉血管造影检查中的应用价值。方法:60例患者按就诊时间顺序随机分成两组,每组30例,均使用双期对比剂注射。A组采用固定对比剂注射法;B组根据体重13s对比剂注射法。分别比较各组冠脉分支和评分,测量升主动脉根部和左心室的CT值,以CT值作为对比增强强化程度。结果:各组对比剂总量平均为:A组75.0ml,B组52.2ml。B组较A组减少约30.4%(P<0.01);各组对比剂流速(ml/s)为:A组5.0ml/s,B组4.0ml/s。B组较A组降低约19.7%(P<0.01)。主动脉根部、左心室和室间隔的CT值差异无统计学意义(P>0.05)。各组冠状动脉增强水平均能满足临床诊断,冠脉图像质量得分差异无统计学意义(P>0.05)。结论:根据体重对比剂注射方案对比剂用量少,有利于减少对比剂不良反应和并发症的发生。

Objective： To evaluate the application value of the contrast media injection protocol in 64-slice spiral CT coronary angiography（CTCA） examination.Methods： 60 patients underwent CTCA examination with 64-slice CT were randomly divided into two groups,30 patients in each group.All patients were injected by using biphasic injection protocol,with traditional injection protocol in group A,with 13 seconds contrast media injection protocol in group B.The coronary artery branches and scores in patients in 2 groups were compared respectively,the CT values of aortic root and left ventricle were measured,and the quality of the coronary artery image was reinforced based on the CT values.Results： The total contrast media volumes（ml） and injection speed（ml/s） in group A and B were 75.0 ml,5.0 ml/s and 52.2 ml,4.0 ml/s respectively,30.04% and 19.70% lower than those in group A in volume and speed respectively（P0.01）.There were no statistic differences with the CT values in aortic root,left ventricle and interventricular septum（P0.05）.The enhencement level of coronary arteries is able to meet the need of clinical diagnosis in each group,but the image quality scores of coronary artery in each group were no difference（P0.05）.Conclusion： The dose tailored to patient body weight in the injection protocol may be helpful to reduce the side effect of contrast media and the patients＇ complication in CTCA examination.
