椎弓根螺钉内固定及椎间植骨融合治疗退行性腰椎滑脱疗效

Curative effect of posterior lumbar interbody fusion combined with pedicle screw fixtation on degenerative lumbar spondylolisthesis

目的探讨经后路减压、椎间植骨融合加椎弓根螺钉内固定治疗老年性退行性腰椎滑脱症的临床疗效。方法本组28例经保守治疗无效的腰椎滑脱患者,采用椎板开窗减压+椎间、椎旁植骨+椎弓根螺钉内固定手术治疗。结果 28例术后随访6～24个月,内固定位置均良好,临床疗效优15例,良7例,一般5例,差1例,有效率78.6%。术后滑脱不同程度改善。结论后路减压、椎间植骨加椎弓根螺钉内固定是治疗退行性腰椎滑脱的一种安全有效的方法,临床疗效满意。

Objective To evaluate the clinical effectiveness of posterior lumbar decompression with posterior lumbar interbody fusion and pedicle screw fixation in senile patients with degenerative lumbar spondylolisthesis.Methods Twenty-eight cases of senile degenerative lumbar spondylolisthesis who were ineffective with conservative therapy were treated with spinal canal decompression,posterior lumbar interbody and intertransverse fusion and pedicle screw fixation.Results All cases were followed up for six to twenty four months,fifteen cases were excellent,seven cases were good,five cases were fair and one case were ineffective which were evaluated with Steffee grade.The effective rate was 78.6%.Conclusion Posterior spinal canal decompression,lumbar interbody fusion combined with pedicle screw fixtation is an effective method to treat senile lumbar spondylolisthesis.
