三种不同方法治疗宫颈糜烂的临床效果

Clinical effect of three different methods for treating cervical erosion

目的探讨微波、聚焦超声法和高频电波环切术(LEEP刀)治疗宫颈糜烂的临床效果。方法选择武汉市中心医院宫颈糜烂患者320例,其中轻度80例,随机分配到微波组(微波治疗)和聚焦超声法组(聚焦超声法治疗),每组各40例;中度和重度糜烂各120例,随机分配到微波组、聚焦超声法组和LEEP组(LEEP刀治疗),每组中度或者重度糜烂各40例。评定三组患者治疗后1个月、治疗后2个月和治疗后3个月治愈、好转和无效情况。结果①聚焦超声法治疗后与微波治疗后,各时间段及各种程度的治愈率比较,差异无统计学意义(P<0.05)。②三种治疗方法对中度糜烂的治愈率比较,差异无统计学意义(P>0.05)。对于重度糜烂LEEP治疗的治愈率高于聚焦超声法和微波治疗(P<0.05),而聚焦超声法治疗和微波治疗比较差异无统计学意义(P>0.05)。结论微波和聚焦超声法治疗重度糜烂的疗效较差,LEEP治疗中、重度宫颈糜烂较前二者的治愈率高、愈合时间短,故轻度、中度糜烂可首选微波和聚焦超声法治疗,重度糜烂首选LEEP刀治疗。

Objective To investigate the clinical effects of treating cervical erosion with microwave,focused ultrasound and loop electrosurgical excision forocedure（LEEP）. Methods Three hundred and twenty cases of cervical erosion in Wuhan central hospital were chosen.Among the patients,80 cases of mild erosion were randomly assigned to the microwave（microwave treatment） and focused ultrasound group（focused ultrasound treatment）,40 cases in each group;Both 120 cases of moderate and severe erosions were randomly assigned to the microwave group,focused ultrasound group and the LEEP group（LEEP treatment）,each group with 40 cases of moderate or severe erosion.The heal,improvement and invalid in 1 month,2 months and 3 months after treatment were assessed. Results ① After the focused ultrasound treatment and microwave treatment,compare the time periods and various degrees of cure rate,the difference was not statistically significant（P0.05）.②Compare the cure rate for moderate erosion of three kinds of treatment,the difference was not statistically significant（P0.05）.For the treatment of severe erosion,the cure rate of LEEP treatment was higher than the focused ultrasound and microwave therapy（P0.05）,while the difference between focused ultrasound therapy and microwave therapy was not statistically significant（P0.05）.Conclusion Microwave and focused ultrasound treatment of severe erosion have poor curative effect,LEEP treatment of moderate and severe cervical erosion has higher cure rate and shorter healing time,so microuave and focused ultrasound can be the preferred treatment of mild and moderate erosion,and LEEP knife treatment be the preferred treatment of severe erosion.
