奚九一论治脉管病的新思维

New Thought of Professor Xi Jiuyi in Treating Peripheral Vascular Disease

介绍奚九一用清法治疗脉管病的新思维。与传统活血化瘀法治疗脉管病有别,奚氏认为,脉管病以血热或络热多见,即使是脱疽的急性发作期,多以邪盛(热毒)为主,虽外见一派寒象,但其病证本质为"真热假寒",是热毒炽盛的表现,即所谓"热深厥深"。治疗时应辨清寒热真假,一法或多法并用。对糖尿病足坏疽的研究,除了重视缺血性的血管变性之外,更应重视非缺血性的肌腱变性(筋疽),以开拓糖尿病足的防治新途径,提高临床疗效。

To introduce new thought of professor Xi jiuyi ＂clearing therapy＂ in treating peripheral vascular disease. To differ from traditional therapy method of activating blood flow and removing blood stasis, professor Xi suggested that the blood heat or collateral heat was pathogenesis mechanism of peripheral vascular disease mostly .even in the acute stage, It was still due to excessive pathogenic（heat and toxin）.also it appeared all of cold syndrome, but ＂pseudofrigid heat＂ was the really nature, It was based on excessive heat and toxin, so called ＂excessive heat cause excessive syncope＂. The true nature of cold and heat should be differentiated clearly and one method or more methods should be used when it was be treated. On study of diabetic foot gangrene, excepting emphasizing on ischemia vascular disease, the nonischemia tendon degeneration and necrosis（tendon gangrene）should be pay more attention to, so the new way of prevention and treating diabetic foot could be developed, the clinical therapeutic effect could be enhanced.
