多排螺旋CT曲面重建在低位胆道梗阻诊断中的应用

摘要 目的评价多排螺旋CT曲面重建(CPR)技术在低位胆道梗阻诊断中的临床应用和价值。方法 90例低位胆道梗阻患者行薄层CT扫描,沿胆道或胰管路径进行曲面重建,以手术、病理及临床随访结果为诊断标准。结果 CPR的定位诊断准确率为100%;病因诊断准确率为93.3%,其中良性梗阻的准确率为95.5%,恶性梗阻的准确率为87.0%。结论 CPR在一幅图像上完整显示胆管和胰管的全貌,同时清晰显示梗阻部位、病灶与胰胆管和周围比邻组织的关系,是横断位图像和多平面重建(MPR)图像的重要补充,具有很好的诊断作用和临床应用价值。 Objective To evaluate the clinical application and value of multislice helical CT curved planar reformations(CPR)in diagnosis of low-set biliary obstruction.Methods Ninety patients of low-set biliary obstruction were included in this study.Thin slice scans were used to acquire images,and CPR technique was used to reconstruct the images along the direction of pancreatic or bile ducts distribution.The surgery,pathology and clinical follow-up result were regarded as gold standard.Results The accuracy of CPR on localizing diagnosis was 100%,the accuracy of qualitative diagnosis was 93.3%,among the benign obstruction was 95.5% and the malignant obstruction was 87.0%.Conclusion CPR technique is obtained to completely display the full view of biliary tract and pancreatic duct on one image,meanwhile clearly illustrate the obstructive location,pancreatic and bile ducts relationship between to mass lessons and the relationship between mass lessons with ambient tissue.CPR image is important addition of axial and MPR images,has satisfactory diagnostic effect and clinical application value.
