菌阴肺结核患者血清SP-A检测及其相关因素分析 被引量:4

摘要 目的探讨肺泡表面活性物质相关蛋白A(SP-A)水平改变对菌阴肺结核的诊断价值。方法采用ELISA法检测51例菌阴肺结核患者(观察组)、85例健康人(对照组)血清SP-A水平,并对观察组血清SP-A水平与其他指标(性别、年龄、PPD试验、结核抗体、血沉、白细胞计数、中性粒细胞比例、淋巴细胞比例、胸部CT等)行Spearm an秩相关性分析。结果观察组SP-A水平明显高于对照组(P<0.05),SP-A水平与PPD、结核抗体、血沉、淋巴细胞、中性细胞等均无相关性。结论血清SP-A可作为反映肺泡毛细血管膜屏障功能损伤情况早期而敏感的外周血标志物,检测其水平有助于菌阴肺结核的诊断。 Objective To explore the value of serum surfactant protein-A ( SP-A ) for the diagnose of pulmonary tuberculosis. Methods The level of serum SP-A was detected by ELISA method in 51 patients with smear and culture negative pulmonary tuberculosis (observation group) and 85 healthy control (control group), additionally, the correlation between the SP-A and other factors ( gender, age, PPD trail, tubercle bacillus antibodies, erythrocyte sedimentation rate, leucocyte count, and neutrophilic granulocyte percentage, lymphocyte percentage, and thorax CT) were analyzed by the spearman rank correlation. Results The level of serum SP-A in the observation group was much higher than that in the control group( P 〈 0.05 ), and no correlation were found between the SP-A and PPD trail, tubercle bacillus antibodies, erythrocyte sedimentation rate, leucocyte count, neutrophilic granulocyte percentage, lymphocyte percentage and so on. Conclusions The serum SP-A can be taken as an early and susceptible marker in the peripheral blood, for reflecting the function of alveolar capillary membrane, the detection of serum SP-A is of great value for the diagnose of smear and culture negative pulmonary tuberculosis.
