实时三维超声心动图在主动脉无缘的房间隔缺损封堵术中的应用

Application value of real-time three dimensional echocardiography in closure of atrial septal defect with absent aortic rim

目的探讨实时三维超声心动图在经导管封堵治疗主动脉无缘的房间隔缺损(ASD)的应用价值。方法 21例主动脉无缘的ASD患者术前均经经胸超声心动图、实时三维超声心动图(RT-3DE)检查,观察ASD的大小,形态,边缘情况,并对无缘范围进行估计,术中监测封堵,术后进行随访。结果 RT-3DE测量ASD大小,估计无缘范围方面优于二维超声心动图,RT-3DE显示ASD形态为椭圆形19例,均封堵成功,2例失败为类圆形患者。结论实时三维超声心动图在主动脉无缘的房间隔缺损封堵术中更具有优越性。

Objective To investigate the application value of real-time three dimensional echocardiography（RT-3DE） in closure of ASD associated with absent aortic rim. Methods Twenty-one patients with ASD associated with absent aortic rim were examined by RT-3DE and TIE before operation, the sizes, shapes, rims and sites of ASD were measured, monitored and guided by RT-3DE during the operation; then were followed-up after the operation. Results RT-3DE showed 19 ASDs associated with absent aortic rim were approximately ellipse. There were differences between RT-3DE and TIE in maximal diameter of ASD and estimation the range of absent aortic rim. The 19 ASDs associated with absent aortic rim got successful occlusion by the monitoring and guiding of RT-3DE; 2 patients with circular ASDs were unsuccessful. Conclusion RT-3DE may accurately measure the sizes and the shapes, has more important clinical value in closure of ASD with absent aortic rim.
