不同术式在子宫全切的临床比较

Uterine total resection of the clinical effect of different operative methods for comparison

目的探讨不同术式子宫全切的临床效果。方法回顾性分析368例子宫全切术患者的临床资料,其中行传统腹式子宫全切术(TAH)128例、阴式子宫全切术(TVH)67例、腹腔镜辅助阴式子宫全切术(LAVH)88例、腹腔镜下全子宫切除术(LTH)85例,比较4组患者术中、术后相关指标情况。结果子宫全切不同术式所耗费的手术时间、术中出血量存在显著差别(P<0.05);术后发热率、应用抗菌素时间、胃肠功能恢复时间、平均住院日期及住院费用等存在显著差异(P<0.05)。结论采取不同术式进行子宫全切效果各有利弊,且相互不能完全替代,应根据医、患双方具体情况进行综合分析,再选择适宜的手术方式。

Objective To explore different operative methods for the clinical effect of total resection of the uterus.Methods Retrospective analysis of 368 patients all patients with uterine cut method of clinical data,including do traditional abdominal hysterectomy full cut method（TAH） 128,Yin type uterine all cut method（TVH） 67 cases,laparoseopic hysterectomy auxiliary Yin type all cut method（LAVH） 88,laparoscopic hysterectomy（LTH） all 85 cases of patients,compares four groups perloperative and postoperative relevant indicators.Results The uterus total resection of the different operative methods which takes operative time,peri-operative bleeding significant difference（P0.05）.Postoperative heating rate,application of antibiotics time and recovery of gastrointestinal function time,average inpatient date and hospitalization expenses there was significant difference（P0.05）.Conclusion And take different operative methods for uterine full cut effect each have advantages and disadvantages,and mutual do not replace,should according to the medical,suffer from both sides specific situation analysis,then choose the suitable operative method.
