芬太尼透皮贴剂与口服吗啡控释片治疗转移性骨肿瘤疼痛的疗效比较

Efficacy of Fentanyl Transdemal Patch vs. Oral Morphine Control Release Tablet for Pain Caused by Metastatic Bone Tumor

目的:比较芬太尼透皮贴剂与口服吗啡控释片在治疗骨转移疼痛患者中的镇痛效果和不良反应。方法:将146例转移性骨肿瘤疼痛患者非随机分为2组,A组使用芬太尼透皮贴,B组口服吗啡控释片,比较2组的疗效、不良反应和对生活质量的影响。结果:2组用药后止痛效果明显,A组总缓解率为93.4%(71/76),完全缓解41例,B组总缓解率为92.9%(65/70),完全缓解37例2,组总缓解率相比差异无统计学意义(P>0.05)。A组头晕、恶心、呕吐、排尿困难等不良反应发生率低于B组(P<0.05),便秘发生率明显低于B组(P<0.01),差异有统计学意义。结论:芬太尼透皮贴剂治疗转移性骨肿瘤疼痛疗效显著,有持久稳定的镇痛作用,使用方便,不良反应小,值得临床推广应用。

OBJECTIVE：To evaluate the analgesic efficacy and adverse drug reactions of Fentanyl transdemal patch vs.oral morphine control release tablet for pain induced by metastatic bone tumor.METHODS：A total of 146 patients with moderate or severe pain induced by metastatic bone tumor were non-randomly assigned to either group A treated with fentanyl transdemal patch or group B with oral morphine control-release tablet.The differences between the two groups in therapeutic efficacy,adverse effects and quality of life were compared.RESULTS：Remarkable analgesic efficacy was achieved for both groups after treatment,with total remission rates of 93.4%（71/76） and complete remission of 41 cases in group A as compared with total remission rates of 92.9%（65/70） and complete remission of 37 for group B（P0.05）.The incidence of the adverse drug reactions including dizziness,nausea vomiting and dysuria was lower in group A than in group B（P0.05） and the incidence of constipation was significantly lower in group A than in group B（P0.01）.CONCLUSION：Fentanyl transdermal patch has remarkable efficacy and mild side effect for osseous metastatic pain,and its analgesic efficacy is long-lasting and stable and it is convenient to use hence it is worthy of recommendation in clinical practice.
