超声引导下颈内静脉穿刺置管的临床应用

Application of ultrasound-guided internal jugular vein access and catheterization

目的探讨超声引导下颈内静脉穿刺中心静脉置管术的应用价值。方法将80例行颈内静脉穿刺中心静脉置管手术患者随机分为超声组(45例)和对照组(35例)。超声组于超声引导下行穿刺,对照组采用传统体表解剖标志定位穿刺。比较两组穿刺时间、成功率和并发症发生率。结果超声组穿刺时间短于对照组[(180±150)svs.(540±210)s](P<0.05),成功率高于对照组(93.3%vs.88.5%)(P<0.05)。超声组无并发症发生;对照组误插动脉造成皮下血肿4例。结论超声引导下颈内静脉穿刺的效果明显优于传统体表解剖标志定位下穿刺。

Objective To investigate the outcomes of ultrasound-guided internal jugular vein access and catheterization.Methods Eighty surgical patients undergoing central venous catheter placement were randomly assigned to two groups of U（ultrasound-guided internal jugular vein access and catheterization,45 cases） and C（classical anatomic landmark technique,35 cases）.The time spent for puncture,success rate and incidence of complications were compared between two groups.ResultsThe time spent for puncture was shorter in group U than that in group C[（180±150） s vs.（540±210） s]（P0.05）.The success rate was higher in group U than that in group C（93.3% vs.88.5%）（P0.05）.No any complication was seen in group U.Subcutaneous hemotoma secondary to incidental arterial puncture occurred in four cases in group C.Conclusion The outcomes of ultrasound-guided internal jugular vein access and catheterization are better than the classical anatomic landmark technique.
