螺旋CT对病毒性肝炎所致肝硬化的分级研究

Grading of severity of viral-induced cirrhosis by using CT imaging

目的探讨螺旋CT对病毒性肝炎所致肝硬化的严重程度分级的可行性、标准及其价值。方法回顾性分析病毒性肝炎肝硬化患者60例,其中15例经病理活检确诊,45例经临床检查确诊。60例患者均行螺旋CT平扫并增强扫描。结果 53例(88.3%)患者的CT分级与临床Child-Pugh(C-P)分级相符,7例(11.7%)患者的CT分级与临床C-P分级不相符,其中5例患者CT分级高于临床C-P分级,2例患者CT分级低于临床C-P分级。CT分级与临床C-P分级呈高度相关(r=0.79,P<0.05)。结论应用螺旋CT对病毒性肝炎所致肝硬化严重程度进行分级是可行的。

Objective To explore the method whether UI imaging can be used to grade the severity ol v：ral-mclucecl cirrhosis. Methods Ventral plain and enhanced helical CT findings of 60 hospitalized patients with viral-induced cirrhosis were analyzed. Fifteen patients of them were improved by pathological diagnosis, and the diagnoses of the others were made according to the standard of viral-induced cirrhosis which was made in China. Results Fifty-three （88.3%） patients were accurately cor- related with clinical Child-Pugh grades of A, B and C. Of the seven （11.7~/~） incorrectly graded patients, five were overesti- mated and two were underestimated. Coefficient of product-moment correlation （r） between the two methods was higher （r~ 0.79, P^0.01）, Conclusion CT grade can be served as a valuable tool for grading the severity of viral-induced cirrhosis,
