摘要
目的探讨剖宫产术后瘢痕子宫再次妊娠对孕产妇及胎儿的影响。方法回顾性分析2009年1~12月在我院住院分娩的有剖宫产手术史孕妇的临床资料。结果 132例瘢痕子宫患者,阴道分娩36例,再次剖宫产96例。术中发现子宫切口隐性破裂11例,严重腹腔黏连34例,产后出血10例。新生儿窒息4例,子宫次全切除1例。结论降低首次剖宫产率,加强孕期保健,术前B超预测瘢痕子宫切口厚度,在严密检测下鼓励瘢痕子宫再次妊娠者阴道试产,可降低瘢痕子宫患者再次妊娠的母婴围产期并发症。
Objective To study the influence of re-pregnancy with the cicatrix uterus on the gravida and fetus.Methods A retrospective study of 132 cases who had received cesarean in our hospital in 2009.Results Of the 132 cases of re-pregnancy with cicatrix uterus,36 chose vaginal delivery and 96 cases received repeated cesarean section.Recessive rupture of uterus occurred in 11 cases and 34 cases were found to have serious pelvic adhesion.Postpartum hemorrhage occurred in 10 cases and newborn suffocation in 4 cases.Subtotal hysterectomy was performed in 1 case.Conclusion To prevent perinatal complications,it is strongly advisable to cut down the first cesarean rate,improve first cesarean skill,reinforce the management of re-pregnancy women with cicatrix uterus,check the thickness of scar by B-ultrasound before operation and encourage vaginal delivery.
