腹腔镜辅助在小切口下胃癌D2根治术中的应用价值

Application of laparoscopic-assisted D2 lymph node dissection for small-incision radical gastrectomy

目的探讨腹腔镜辅助在小切口下胃癌D2根治术中的应用价值。方法回顾分析30例患者行腹腔镜辅助在小切口下胃癌D2根治术的临床资料。结果 30例均顺利完成腹腔镜辅助小切口胃癌D2根治术,无中转开腹。根治性近端胃大部切除8例,手术时间(231±21)min,术中出血量(163±23)mL;根治性全胃切除6例,手术时间(322±25)min,术中出血量(320±28)mL。根治性远端胃大部切除16例,手术时间(203±26)min。结论腹腔镜辅助在小切口下胃癌D2根治术在保留腹腔镜手术微创优点的同时降低了全腹腔镜胃癌根治术的手术难度,获得开腹手术相当的临床疗效。具有良好的临床应用价值。

Objective To investigate the clinical value of laparoscopic assisted D2 lymph node dissection for radical gastrectomy under small-incision. Method The clinical data of thirty consecutive patients with advanced gastric carcinoma were analyzed retrospectively. Result All laparoscopic assisted D2 lymph node dissection for radical gastrectomy were successfully performed and no case was transferred to open surgery. Among the 30 cases,8 underwent radical proximal gastrectomy in which the operation time, blood loss were （231±21） min and （163±21） ml respectively. Six cases received radical total gastrectomy and both operation time and blood loss were（322±25）min and （320±28） ml respectively. And 16 cases underwent distal gastrectomy and operation time was （203±26） min. Conclusion Laparoscopic-assisted small incision in the D2 resection of gastric cancer not onlu reduce difficulties, but also minimize the surgical injury.
