摘要
目的比较肺癌常规放射治疗和三维适形放射治疗两种不同放射治疗方案中靶区及正常组织的受量。方法应用拓能公司三维治疗计划系统在20例肺癌患者的CT图像上设计三维适形放射治疗计划,然后在CT图像上模拟常规射野照射。总剂量均给60 Gy左右。比较两种计划肺、脊髓等正常组织受量。结果常规照射和适形放射治疗相比较,肿瘤体积各项指标有显著性差异,脊髓的最大受照剂量、肺的V20有明显的差异(P<0.05)。结论三维适形放射在靶区覆盖度、靶区适形度上均优于常规照射计划,且能减少肺的受照体积百分比,减少放射性肺炎的发生。
Objective To compare the dose distribution of conventional and three-dimentional radiotherapy(3DCRT)in the planning target volume and organs at risk in treating lung cancer.Methods Conventional radiotherapy and 3DCRT treatment planning were performed in the 3D treatment planning system for 20 patients with lung cancer.Dose volume histogram(DVH)was used to compare the dose distribution in the CTV,lungs and spinal cord.Total given dosage both around 60 Gy.Results There was significant difference between the two techniques in terms of the dose coverage of CTV.The volume of both lungs receiving at least 20 Gy(V20)were significantly different by the two techniques(P0.05).Conclusion Compared to the conventional radiotherapy technique,3DCRT has better target coverage and target shape comformity,reducing the covered volume percentage of the lungs,and decreases the radiation induced pneumonitis possibilities.
