摘要
目的观察智能电子分光技术(FICE)在Barrett食管(BE)及早期食管腺癌(EA)诊断中的价值。方法 2006年6月~2008年12月该院胃镜检查的患者16000例次,对其食道下端随机采用传统白光或FICE技术观察。FICE内镜检查组拟诊BE133例(实验组);普通内镜检查组拟诊BE128例(对照组),对检查部位进行活检,并与组织学诊断进行对比。结果实验组确定BE127例(95.5%),其中轻度不典型增生(LGD)13例(9.77%)、高度不典型增生(HGD)10例(7.51%)、EA9例(6.02%);对照组确定BE105例(82.0%),其LGD10例(7.81%)、HGD3例(2.45%)、EA2例(1.56%),实验组除LGD外,其余检出率均明显高于对照组(P<0.05)。结论 FICE对BE病灶发现及指导活检优于普通白光内镜。
[Objective] To investigate the clinical value of Fujinon intelligent chromoendoscopy(FICE) in the diagnosis of Barrett's esophagus(BE) and esophageal adenocarcinoma(EA).[Methods] All the patients who received endoscope were randomly divided into two groups.From June,2006 to December,2008,133 patients,who accepted FICE and thus were detected suspected as BE patients,served as experiment group;another 128 patients,who accepted common endoscopy and then were detected as BE-suspects,served as control group,pathological samples were compared.[Results] 127 cases(95.5%) of BE were identified in the experiment group,among which there are 13 cases(9.77%) of low-grade dysplasia,10 cases(7.51%) of high-grade dysplasia,9 cases(6.02%) of esophageal adenocarcinoma.105 cases(82.0%) of Barrett's esophagus were identified in the control group,among which there are 10 cases(7.81%) of low-grade dysplasia,3 cases(2.45%) of high-grade dysplasia,2 cases(1.56%) of esophageal adenocarcinoma.[Conclusions] FICE is superior to ordinary white light endoscopy in detecting BE and EA.
