YLD-200通液诊疗仪在“T”形管胆道造影中的临床应用价值

Clinical application value of YLD-200 coherent-liquid and diagnosis-treatment machine using in T-tube retrograde cholangiography

目的探讨YLD-200通液诊疗仪在"T"形管胆道造影中的临床应用价值。方法将96例因各种原因进行胆系手术后需行"T"形管胆道造影的患者随机分配为常规组(48例)和试验组(48例),常规组由医生穿铅衣站在检查台旁直接暴露在X线下使用手工推注造影剂法进行造影,试验组由医生在工作室(无射线区)通过操控YLD-200通液诊疗仪控制器以代替手工推注造影剂法进行造影,对两组造影所显示的图像质量和在造影过程中可能发生的不良反应进行比较。结果医生不用直接暴露在X线下使用YLD-200通液诊疗仪亦可进行"T"形管胆道造影。根据统计,在图像质量方面,常规组甲级片41例、乙级片5例、丙级片2例,试验组甲级片43例、乙级片4例、丙级片1例,两组间图像质量比较差异无统计学意义(uc=0.626,P>0.05);而在造影过程中所发生的不良反应,常规组无反应32例、轻度10例、中度5例、重度1例;试验组无反应44例、轻度3例、中度1例、重度0例,试验组明显低于常规组(uc=3.022,P<0.01)。结论 YLD-200通液诊疗仪亦可适用于"T"形管胆道造影,且安全、可靠,同时可避免医生遭受X射线的辐射,值得临床推广使用。

Objective To investigate clinical application value of YLD-200 coherent-liquid and diagnosis-treatment machine using in T-tube retrograde cholangiography.Methods 96 patients of biliary tract surgery for various reasons with T-tube retrograde cholangiography were randomly divided into the routine group（n=48） and the experiment group（n=48）.T-tube retrograde cholangiography was performed in the routine group by the doctor who wore lead clothes and who was directly exposed after X-ray irradiation to manual bolus at the bedside.T-tube retrograde cholangiography was performed in the experiment group by the doctor who operated YLD-200 coherent-liquid and diagnosis-treatment machine to replace manual bolus in studio（no X-ray area）.The image quality of two groups was compared to see whether there was difference,and the adverse reaction of two groups in T-tube retrograde cholangiography was compared to see whether there was difference.Results The doctors were not directly exposed after X-ray irradiation to perform T-tube retrograde cholangiography by YLD-200 coherent-liquid and diagnosis-treatment machine.According to statistics,in the routine group,the quality of the image was classified into grade A in 41 cases,grade B in 5 cases and grade C in 2 cases.In the experiment group,the quality of the image was classified into grade A in 43 cases,grade B in 4 cases and grade C in 1 case.There was no significant difference in image quality between two groups（uc=0.626,P〈0.05）.Results of the adverse reaction in the routine group showed no effect in 32 cases,slight in 10 cases,medium in 5 cases and severe in 1 case.Results of the adverse reaction in the experiment group demonstrated no effect in 44 cases,slight in 3 cases,medium in 1 case and severe in 0 case.The adverse reaction of two groups in T-tube retrograde cholangiography had significant difference（uc=3.022,P〈0.01）.Conclusion YLD-200 coherent-liquid and diagnosis-treatment machine can be used to T-tube retrograde cholangiography.It is safer and dependable,and it can effectively prevent the doctor from X-ray radiation.It is worth popularizing and use in clinic.
