血浆胱抑素C浓度与非慢性肾病冠心病患者冠状动脉病变严重程度的相关性 被引量:1

Correlation between Plasma Cystatin C Concentration and the Severity of Coronary Artery Disease in Patients without Chronic Kidney Disease

摘要 目的观察胱押素C(cystatinC,CysC)浓度是否能反映非慢性肾病冠心病(coronaryarterydisease,CAD)患者冠状动脉病情严重程度。方法检测106例行冠状动脉造影术患者血浆中CysC及其相关指标的浓度.根据血清肌酐(SCr)浓度估算肾小球滤过率(glomerularfiltrationrate,GFR),以GFR60ml/min·1.73m^2为界值,将患者分成慢性肾病组(chronickidneydisease,CKD)和非慢性肾病组;在非CKD组中,以CysC浓度0.9mg/L为界值,分为正常浓度组和高浓度组,并将检测结果进行比较。结果CKD组患者的CysC浓度以及CAD的发病丰与非CKD组相比有显著性差异(f=6.37,x^2=6.88,P〈0.01)。非CKD组中高CysC浓度组与正常CysC浓度组CAD的发病率差异有统计学意义(r=4.21,P〈0.05)。非CKD组CAD患者病情的严重程度与CysC浓度呈正相关0=0.684,P〈0.01)。结论GFR正常的CAD患者,CysC浓度升高与CAD的病情严重程度相关。对于非CKD的CAD患者来说,CysC很可能是监测CAD病情发展的标志物。 Objective To study the correlation between the concentration of plasma cystatin C (Cys C) and the severity of coronary artery disease (CAD)in patients without chronic kidney disease (CKD). Methods The plasma Cys C and other indexes of 106 patients who underwent coronary angiography were detected. Glomerular filtration rate (GFR)was calculated by serum creatinlne (SCr). Based on GFR 60 ml/min ·1.73m^2,the patients,were divided into with and without CKD group. Based on Cys C 0.9 mg/L,the patients of non-CKD group were divided into normal and high level group,and then the results from different group were compared. Results The level of Cys C and the incidence of CAD were significantly different between CKD and non-CKD group (t= 6.37, X^2 = 6. 88 ,P〈 0. 01). Compared with normal Cys C group, the incidence of CAD were significantly higher than that of increased Cys C group (x^2= 4. 21,P〈0. 05) in non-CKD group. Cys C concentration demonstrated positive correlation with the severity of CAD (r= 0. 684 ,P〈0.01) in non-CKD group. Conclu- sion Among CAD patients with normal plasma GFR,the high level of Cys C corelated with the severity of CAD. Cys C might serve as a marker of CAD severity for the CAD patients of non-CKD.
