摘要
目的:分析高血压患者血压晨峰与Q-T离散度的关系。方法:对156例高血压患者做24 h动态血压监测及同步12导联心电图检查。根据动态血压监测情况分为两组,晨峰组(86例),血压晨峰(起床后2 h内平均收缩压-睡眠时最低平均收缩压)≥55 mm Hg(1 mm Hg=0.133 kPa),其他为非晨峰组(70例)。同步12导联心电图测量Q-T间期(Q-T),校正Q-T间期(Q-Tc),Q-T离散度(Q-Td),校正Q-T离散度(Q-Tcd)。结果:晨峰组与非晨峰组Q-Tc,Q-Td,Q-Tcd比较,差异有高度统计学意义(P<0.01)。结论:高血压有晨峰现象的患者,Q-Tc,Q-Td,Q-Tcd显著延长,心室肌电不稳定性增加。
Objective: To analyze the relationship between morning blood pressure surge arid Q-T dispersion of hypertension patients. Methods: 156 patients with hypertension were performed by 24 h ambulatory blood pressure monitoring and surface 12 electro-eardio graphic lead. They were divided into with morning blood pressure surge group (86 cases) and without morning blood pressure surge group (70 cases) according to the condition of 24 h ambulatory blood pressure monitoring. Mean systolic blood pressure during the 2 h after awaking minus the lowest average systolic blood pressure during sleep was the blood pressure surge, and the morning blood pressure surge≥55 mm Hg (1 mm Hg=0.133 kPa). Q-T interval (Q-T), heart-rate corrected Q-T interval (Q-Tc), Q-T dispersion (Q-Td), heart-rate corrected Q-T interval dispersion (Q- Ted) were measured by 12 electro-cardio graphic lead. Results: Q-Tc, Q-Td, Q-Tcd of morning blood pressure surge group were significantly higher than without morning blood pressure surge group (P〈0.01). Conclusion: Q-To, Q-Td, Q--Tcd and the electrophysiologieal instability of ventricular myocardium of the patients with hypertension in morning blood pressure surge increase significantly.
