真菌性副鼻窦炎的CT诊断

CT diagnosis of the fungal sinusitis

目的:分析真菌性副鼻窦炎的CT表现和特征,探讨CT在该病临床诊断中的价值。方法:回顾性分析27例经手术及病理证实的真菌性副鼻窦炎的CT表现。结果:单侧上颌窦炎18例,双侧同时发病的5例(其中1例累及左侧筛窦),筛窦3例,蝶窦1例。CT表现主要为25例窦腔内有密度较高的不均匀软组织密度影,其中,19例伴有斑点、斑块状及不规则条带状钙化,4例病灶内有散在"小空泡"征,2例病变完全钙化;21例窦壁骨质增厚硬化,2例筛板增厚,窦腔均有所缩小;8例上颌窦内侧壁骨质破坏吸收变薄或中断。结论:真菌性副鼻窦炎的CT表现具有特征性,CT对该病具有较高的诊断价值。

Objective： To analyze the CT performance and characteristics of fungal sinusitis, in order to explore the signifi- cance of CT in the clinical diagnosis of this disease. Methods： CT manifestations of 27 cases which were diagnosed by surgery and pathology as fungal sinusitis were retrospectively analyzed. Results： There were 18 cases of unilateral maxillary sinus, 5 cases bilateral （1 case involved the left ethmoid sinus）, 3 cases of ethmoid sinus, and 1 case of sphenoid sinus. CT manifestations, 25 cases in the high density inhomogeneous sinus soft tissue density, of which 19 cases with spots, patchy and irregular calcification banded, 4 lesions were scattered within the ＂small bubble＂ sign, 2 cases of calcified lesions completely; Sinus walls＇ bones of 21 cases were thickened and hardened, sieves of 2 cases were thickened, their sinus were narrowed; maxillary sinus walls of 8 cases were thinned or interrupted because of absorption of bone destruction. Conclusion： Fungal sinusitis has characteristic CT manifestations, CT diagnosis of the disease has a high value.
