压疮干预指导组在医院压疮管理中的作用

Effect of Pressure Ulcer Intervention and Guiding Group on hospital ulcer management

目的探讨压疮干预指导组在医院压疮管理中的作用。方法成立压疮干预指导组,每月1～6日检查1次全院各科使用Bra-den计分表(压疮危险计分)的及时性和准确性,针对使用中存在的问题进行改良;Braden计分≤12分者,各科负责护士需于24 h内通过院内网络上报压疮干预指导组,组长指定组员24 h内跟踪检查、指导、帮带,及时跟踪发现问题并及时解决,资料登记造册,持续改进;每月20日小组工作结果报护理部,并在全院护士长会公布。结果压疮干预指导组成立后全院压疮高危患者上报例数显著增加(P<0.01),高危患者院内压疮发生率显著下降(P<0.01),正确预防压疮率显著增加(P<0.01)。结论压疮干预指导组在医院压疮高危筛选、上报、指导帮带中发挥了专业作用。

Objective To explore the effect of the Pressure Ulcer Intervention and Guiding Group（PUIGG） on hospital ulcer management.Methods The PUIGG was established.The group checked the timeliness and the accuracy of Braden scoring table once every month from 1st to 6th and provided solutions to existing problems.Once the Braden scoring was detected lower than 12,the charge nurses in local departments should report to PUIGG online within 24 hours.The group leader would assign a specific member to track the ulcer risks,instruct and teach charge nurses to better handle the risks within 24 hours.Through constant follow-up,problems were discovered and solved in time.All the data were collected and recorded for future improvements.On the 20th of every month,the achievements of PUIGG would be reported to nursing department and made public at head nurses meeting.Results With the help of PUIGG,there was a significant increase in the numbers of identified cases at high risk of ulcer（P0.01）,with a significant decrease of incidence of ulcer in these patients（P0.01）.Successful pressure ulcer prevention rate was statically increased（P0.01）.Conclusion The PUIGG plays a professional role in high-risk screening,reporting and instruction.
