小儿麻疹合并细菌性肺炎患者中性粒细胞CD64表达水平的临床应用价值

Clinical value of neutrophil CD64 level in children patients of measles complicated with bacterial pneumonia

目的探讨中性粒细胞CD64的表达在小儿麻疹合并细菌性肺炎感染中的临床应用价值。方法选取麻疹患者133例,按临床表现和实验室检测结果分成麻疹合并细菌性肺炎组及单纯麻疹病毒或其它病毒性肺炎组,应用流式细胞术测定中性粒细胞CD64水平,同时测外周血C反应蛋白(CRP)和白细胞数。结果细菌组CD64水平为(22.15±11.07)MFI,明显高于病毒组的(8.55±2.88)MFI(P<0.01),以CD64≥12.85MFI、CRP≥8.95mg/L、WBC≥9.95×109/L为临界值,3种指标的敏感度分别为86.12%、87.48%、59.37%,特异度分别为76.19%、66.67%和64.28%,准确度分别为77.35%、74.52%和61.32%。结论中性粒细胞CD64的表达水平可作为小儿麻疹合并细菌性感染的诊断、判断病情的可靠指标。

Objective To explore the clinical value of neutrophil CD64 level in children patients of measles complicated with bacterial pneumonia. Methods 133 patients of measles were divided into two groups by clinical manifestation and laboratory examination. Bacteria group were patients of measles complicated with bacterial pneumonia. Virus group were patients of measles only or complicated with viral pneumonia. The neutrophil CD64, C reactive protein （CRP） and white blood cell （WBC） count were determined by flow cytometry. Results The expression of CD64 in the patients of bacteria group was （22.15± 11.07） MFI, which was significantly higher than that in the patients of virus group （（8.55±2.88） MFI） （P〈0.01）. It was considered to be positive if CD64 ≥12.85 MFI, CRP≥ 8.95mg/L and WBC ≥ 9.95×10^9/L. Their sensitivity was 86.12%, 66.67% and 64.28%. Their specificity was 77.35%, 74.52% and 61.32%. Their accuracy rate was 77.35%, 74.52%, 61.32%. Conclusion Neutrophil CD64 level can be regarded as a dependable indicator in the early diagnosis and illness evaluation of children patients of measles complicated with bacterial pneumonia.
