卡前列甲酯栓对无痛人流术中患者血流动力学的影响

Impact of Carboprost Methylate Suppositories on Hemodynamic in Painless Induced Abortion Patients

目的观察术前使用卡前列甲酯栓(卡孕栓)对减轻无痛人流术中患者心血管反应的效果。方法将420例行无痛人流术患者随机分为观察组和对照组,每组210例。2组均采取丙泊酚静脉无痛人流术,观察组在术前60 min经直肠放置卡孕栓,对照组不放。监测记录患者术前(T0)、静脉麻醉后(T1)、放置宫颈扩张器时(T2)、行宫腔负压吸引时(T3)、手术结束时(T4)血压、心率,并记录手术历时、出血量及使用丙泊酚总量。结果 2组T0、T1血压、心率相比差异无统计学意义(P>0.05);T2、T3、T4时间点对照组血压、心率低于观察组,差异有统计学意义(P<0.05);对照组手术时间、出血量、丙泊酚使用量高于观察组,差异有统计学意义(P<0.05)。结论无痛人流术前使用卡孕栓可以减轻术中心血管反应、缩短手术时间、减少出血量。

Objective To study the effect of carboprost methylate suppositories on cardiovascular re- sponses during the painless induced abortion. Methods 420 patients for painless induced abortion were randomly divided into observation group （ group O, n = 210） and control group （ group C, n = 210）. Both groups were injected propofol for painless induced abortion. In group O, earboprost methylate supposito-ries were placed in patients＇ rectum 1 h before the operation. But it was not used in group C. The writers monitored and recorded the patients＇blood pressure,heart rates when beginning operation（T0） , after in- travenous anesthesia（T1 ）, placing cervical dilator（T2）, uterine suction （ T3 ） and the end of operation （T4）. The length of operation, the amount of bleeding and the total propofol dose were also recorded for each patient. Results The differences of blood pressure and heart rates in both groups when TO, T1 was not statistically significant（P 〉0.05） ; however,in group C they were lower than in group O when T2, T3, T4 （P 〈 O. 05）. Comparing with group O, the length of operation, amount of bleeding and the total propofol dose in group C were increasing, and the differences were significant（ P 〈 0.05 ）. Conclusion Using carboprost methylate suppositories before painless induced abortion can alleviate the cardiovascular responses in operation. And the length of operation, the amount of bleeding are also decreased.
