250例小儿上消化道疾病胃镜检查结果分析

Characteristics of Ultra-Slim Electronic Gastroscope of 250 Children with the Upper Alimentary Tract Disease

目的对我院就诊250例有腹痛等消化道症状小儿的临床特征和胃镜检查结果进行分析总结,从而阐示超细电子胃镜检查在小儿上消化道疾病中的诊断价值。方法 250例小儿均行电子胃镜检查,其中26例于胃窦部及十二指肠病变部位取黏膜组织行病理检查,42例胃镜证实存在食管、胃或十二指肠溃疡的病例,均在胃窦部取黏膜组织进行幽门螺杆菌检测。结果 16例为十二指肠球部溃疡,19例腹型过敏性紫癜,5例非甾体抗炎药相关性溃疡,其中1例引起胃腔狭窄,1例食管溃疡并非萎缩性胃炎,1例为反流性食管炎并胃窦部溃疡,1例为白血病患儿发生移植物抗宿主病,引起十二指肠炎,在胃镜下取异物2例,190例为慢性表浅性胃炎或十二指肠炎,15例胃镜下未见异常发现。42例上消化道溃疡幽门螺杆菌检测结果,19例呈阳性反应,主要见于十二指肠球部溃疡者。结论超细电子胃镜检查对儿童是安全可行的,对于小儿上消化道疾病的诊断具有重要价值。

Objective Analysed clinical and endoscopic characteristics of 250 children with abdominal pain and other alimentary tract symptoms,to explore gastroscope diagnostic value in the upper alimentary tract disease.Methods 250 children were examined by gastroscope,26 patients among them were taken biopsy,42 patients with ulcer were taken helicobacter pylori detection.Results 16 patients were duodenal bulb ulcer,19 patients were abdominal anaphylactoid purpura,5 patients were non-steroidal anti-inflammatory drugs induced gastric antrum ulcer,even 1 patient induced outflow stricture.1 patient was esophagus ulcer and non-atrophic gastristis,1 case was refulx esophagitis with gastric antral ulcer.1 case with graft versus host disease was duodena inflammation,2 patients were took foreign body by gastroscope.190 patients were gastric or duodenal chronic inflammation.15 cases were normal.41 patients were detected about helicobacter pylori,19 patients was positive,who were principal duodenal bulb ulcer.Conclusion Gastroscope examination is safe and feasible to children,which shows diagnosis value in upper alimentary tract disease of children.
