Clinical Exploration of Minimal Invasion in Oral Implantology

目的:初步对微创技术在口腔种植中的应用进行临床探讨。方法:2008年至2009年间,共对16例牙缺失患者行微创牙种植术,其中男性6例,女性10例。采用曲面断层全景片和CT计算机导航技术进行术前检查,运用或不运用手术导板,采用不翻开粘骨膜瓣或改良的不翻瓣方法植入种植体。通过测定术后当日、1个月、3个月、6个月、12个月和18个月的x线牙槽骨改变进行临床评价,并对种植体的稳定性进行共振频率分析(RFA),记录分析种植体稳定性数值(ISQ)。结果:共植入种植体57枚,术后8～16周进行永久修复,全部修复获得令人满意的功能和美观效果。随访时间为18个月,种植体存留率为100%,X线观察牙槽骨无明显吸收状况。种植体植入当日ISQ测定为一例患者6枚种植体44-46,其余种植体ISQ大于60,所有种植体冠修复后ISQ值均大于60,经方差分析在各时间段的ISQ值无显著差异。结论:微创牙种植技术操作简单,手术时间短,手术创伤小,在严格地掌握适应症,术前结合CT制定治疗计划,术中仔细操作下,可以获得满意的临床结果。

Objective： To explore the clinical results of dental implants using minimal invasion surgical technique. Methods ： The study population consisted of 16 patients （6 males and 10 females） who were referred implant placement using minimal invasion surgical technique during 2008 - 2009. Computer tomography had been taken before surgery, implants were placed using flapless surgical technique. The clinical parameters were evaluated by checking crestal bone changes of X - ray. Implant stability quotient （ISQ） values were measured at placement surgery and 1, 3, 6, 12, 18 months after installation. Results： 57 implants were placed, final prosthesis were finished 8 -16 weeks after surgery, all implants were in good condition. The survival rate after 18 months＇ following -up was 100% , no significant marginal bone loss of peri - implant was observed. The ISQ at placement surgery were all more than 60 except fnr 6 implants of one patient （ ISQ 44 - 46）, the final ISQ after prosthetics all exceeded 60, and there was no significantly difference. Conclusion： The method of dental flapless surgery is simple, the operation time can be shorten greatly and the invasive was mini- mized, a satisfactory clinical resuh can be arrived contributing to careful patient selection,treatment plan design and operation.
