不同通气策略对单肺通气胸腔镜手术患者呼吸力学及血流动力学的影响

摘要 目的探讨不同通气策略对单肺通气(OLV)胸腔镜手术患者呼吸力学及血流动力学的影响。方法将60例ASA分级为Ⅰ~Ⅱ级的行胸腔镜肺叶切除手术患者分为4组,每组各15例。A组:单肺间歇正压(IPPV)常规潮气量通气组;B组:单肺IPPV常规潮气量通气加呼气末正压(PEEP)5cmH2O(1cmH2O≈98Pa)通气组;C组:单肺IPPV低潮气量加PEEP 5 cm H2O通气组;D组:单肺IPPV低潮气量加PEEP 5 cmH2O通气及非通气侧肺持续气道正压(CPAP)通气组。分别于OLV前(T1)、OLV 30 min(T2)、OLV 60 min(T3)和恢复双肺通气(TLV)30 min(T4)4个时间点观察患者气道峰值压(Ppeak)、平均气道压(Pmean)、动脉血气分析、心率(HR)及平均动脉压(MAP)。结果 OLV后各时点B组Ppeak均明显高于其他3组(均<0.01),而A、C及D三组差异均无统计学意义(均>0.05);B组各时点Pmean均明显高于其他3组(均<0.05),而A、C及D组差异均无统计学意义(均>0.05)。在各观察时点,4组HR及MAP差异均无统计学意义(均>0.05)。在OLV后,各组PaO2明显下降(<0.01),D组OLV后时点的PaO2明显高于其他3组(<0.05)。结论在OLV期间应用单肺IPPV低潮气量加PEEP 5cmH2O通气及非通气侧肺CPAP通气可降低Ppeak,改善氧合,而对血流动力学影响不明显。
