阴道炎五联检测试剂盒临床诊断价值探讨

Clinical Value of Quintuple Kit for the Diagnosis of Vaginitis

【目的】探讨LTS-V400阴道炎检测仪五联试剂在细菌性阴道病（BV）诊断中的临床价值。【方法]248例妇科门诊患者阴道分泌物同时用手工Amsel法、革兰染色和瑞氏染色及阴道炎五联试剂进行检测，并对其结果一致性进行对比分析。【结果]248例样本中，手工Amsel阳性率为21．0%，五联检测法阳性率为21．4％，两者比较差异无统计学意义（r=0．08，P〉0．05）。以Amsel法为诊断BV的参照方法，则联合检法诊断BV的灵敏性为88．5％（46／62），特异性为96．4％（189／196），假阴性率为3．1％（6／196），假阳性率为13．5％（7／52）。两种检测方法的一致性评价，k=0．95，具有高度的一致性。五联检测法的β-氨基半乳糖苷酶阳性病例为55例，阳性率为22．2％（55／248），革兰和瑞氏染色检测霉菌或滴虫感染的阳性病例为58例，阳性率为23．4％（58／248），两者比较差异无统计学意义（x^2=0．06，P〉0．05）。【结论】检测仪五联检测法适用于临床快速、准确诊断细菌、真菌、滴虫等类阴道炎检查，可作为诊断的一种常规方法在临床中推广应用。

[Objective] To explore the clinical value of quintuple kit of LTS-V400 instrument for vaginitis detection in the diagnosis of bacterial vaginosis（BV）. [Methods] The vaginal secretions of 248 gynecologic outpatients were simultaneously detected by manual Amsel method, Gram and Wright＇s staining and quintuple kit. The consistency of the results was compared. [Results] Among 248 samples, the positive rate of manual Amsel method and quintuple kit was 21.0% and 21.4%, respectively, and there was no significant difference between them （X^2 = 0.08, P 〉0.05）. With Amsel method as the reference for the diagnosis of BV, the sensitivity, specificity, false negative rate and false positive rate of the combination method was 88. 5% （46/52）, 96. 4% （189/196）, 3.1%（6/196） and 13. 5%（7/52）, respectively. The consistency evaluation showed that two methods had high consistency（k=0.95）. The positive rate of β-amino-galactosidase by quintuple kit and Gram and Wright＇s staining was 22.2 % （55/248） and 23.4% （58/248）, respectively, but there was no significant difference between them（x^2=0.06, P 〉0. 05）. [Conclusion]Quintuple kit is suitable for the rapid and accurate diagnosis of bacterial, fungal and trichomonas vaginitis, so it can be a routine diagnostic method to be generalized in the clinical application.
