二级医院中青年急性脑卒中患者疾病管理的效果观察

目的探讨二级医院中青年急性脑卒中患者进行疾病管理的效果。方法对105例中青年急性脑卒中首次发病患者进行生活方式问卷调查,综合评估危险因素后建立健康档案,制定相应的疾病管理计划,有针对性地实施健康教育和随访,并进行个体化健康指导,6个月后观察并分析疾病管理效果。结果患者不良生活方式得到改变,坚持服药率达93.02%,戒烟率达90.69%,限酒率97.67%,能坚持康复锻炼患者达90.69%,坚持低盐低脂饮食患者达81.39%。结论对中青年急性脑卒中患者实施疾病管理,为降低疾病发病率和复发率、提高患者的生活质量提供了一条新途径。

Objective To investigate the effect to manage the young patients with acute cerebral apoplexy effect.Methods One hundred and five cases of young patients with acute cerebral apoplexy were enrolled in the survey of lifestyle. We established health files after comprehensive evaluation of risk factors and developed disease management programs and health education accordingly and follow-up and health guidance personally. We observed and analysed of the effect of disease management after 6 months.Results Patient compliance rate of drug was 93.02 %, smoking cessation rate was 90.69%, limited alcohol 97.67%, rehabilitation exercise rate was 90.69 %, low-salt and low-fat diet rate was 81.39%.Conclusion Disease management in young patients with acute cerebral apoplexy patients can help to reduce the incidence of disease relapse rates and improve patient quality of life provides in a new way.
