慢性阻塞性肺疾病患者血清IL-17、IL-23水平及临床意义 被引量:38

摘要 目的:探讨白介素-17(Interleukin-17,IL-17)、白介素-23(Interleukin-23,IL-23)在慢性阻塞性肺疾病(Chronic ob-structive pulmonary disease,COPD)发病过程中的临床意义。方法:采用酶联免疫吸附试验(ELISA)法测定慢性阻塞性肺疾病急性加重期(Acute exacerbations of COPD,AECOPD)组、COPD稳定期组(治疗前后)各30人及健康体检者(健康对照组)30人血清IL-17、IL-23浓度;血常规中性粒细胞(N)百分比(%)、肺功能第一秒用力呼气容积占预计值百分比(FEV1%)由检验科及肺功能室测定。结果:①AECOPD、COPD稳定期及健康对照组血清IL-17、IL-23各组间比较差异具有统计学意义(P<0.05);②AE-COPD与COPD稳定期组、健康对照组外周血N(%)组间比较差异具有统计学意义(P<0.05);③AECOPD、COPD稳定期组及健康对照组FEV1%组间比较差异具有统计学意义(P<0.05);④所有COPD患者血清中IL-23与IL-17呈直线正相关(r=0.449,P=0.000<0.001);⑤所有COPD患者血清中IL-17与N(%)呈直线相关(r=0.273,P=0.035<0.05);⑥所有研究对象血清中IL-17与FEV1%呈直线负相关(r=-0.510,P=0.000<0.001)。结论:IL-17、IL-23与COPD的炎症过程及临床表现具有一定的相关性。 Objective:To investigate serum levels and clinical significance of IL-17,IL-23 in chronic obstructive pulmonary disease(COPD).Methods:Serum IL-17 and IL-23 levels were detected in 90 subjects including 60 COPD(30 acute exacerbation and 30 stable) and 30 healthy controls by ELISA.The percentage of blood neutrophils N(%) and the percentage of forced expiratory volume in one second of predicted value(FEV1%) were measured respectively by the department of Clinical Laboratory and the Lung Function Laboratory of Lanzhou University Second Hospital.Results:①IL-17 and IL-23 serum levels were significantly different(P0.05) among AECOPD,stable phase COPD and healthy controls;②N(%) was statistically significant between AECOPD and stable phase COPD(P0.05) as well as between AECOPD and healthy controls(P0.05);③FEV1% was statistically significant(P0.05) among AECOPD,stable phase COPD and healthy controls;④There was a linear correlation between serum IL-17 and IL-23 in all COPD patients(r=0.449,P=0.0000.001);⑤There was a linear correlation between serum IL-17 levels and N(%) in all COPD patients(r =0.273,P=0.0350.05);⑥There was a negative linear correlation between IL-17 serum levels and FEV1% in all subjects(r=-0.510,P=0.0000.001).Conclusion:IL-17 and IL-23 serum levels might be associated with inflammation in COPD and have some relevance to COPD clinical manifestations.
