摘要
目的运用神经网络重建仪治疗脑梗死后足下垂,研究神经网络重建仪的作用机制,探讨其治疗方法。方法脑梗死患者40例随机分为治疗组和对照组。对照组只给予康复训练,治疗组在对照组的治疗基础上加用神经网络重建仪治疗。所有患者在治疗前和治疗4周后均评定患侧踝关节主动背屈角度、患侧小腿三头肌痉挛程度、患侧胫前肌肌力、患侧踝关节背伸时胫前肌的最大收缩电位值(sEMGMax)及用肌电图测定患侧小腿三头肌的H波Max值和M波Max值。结果同组患者在治疗前后的患侧踝关节主动背屈角度s、EMGMax的比较,治疗后较治疗前显著提高(P<0.01);对照组患者治疗前后的CSS评分、HMax/MMax比较,差异具有显著性(P<0.01),而治疗组不具有显著性;两组间治疗后患侧踝关节主动背屈角度s、EMGMax的比较,治疗组较对照组提高的更明显(P<0.01);两组间治疗后的CSS评分、HMax/MMax比较,对照组较治疗组增加明显(P<0.01)。结论神经网络重建仪把低频电刺激和生物反馈技术相结合,通过改善肌力与肌张力来实现治疗足下垂的目的,并对损伤的中枢发挥重塑作用。
Objective To research the mechanism and explore the therapeutic method of Auto Move AM800 for treatment of foot drop of cerebral infarction patients. Methods Forty cerebral infarction patients were randomly divided into treatment group and control group. The control group were only given rehabilitation treatment, and the treatment group were given rehabilitation treatment combined with Auto Move AM800. At the time of pre-treatment and after four weeks, we assessed the range of initiative motion of ankle dorsiflexion, the strength of the anterior tibial muscle, the spasm degree of the calf muscles 3, the sEMGMax of the anterior tibial muscle and the calf muscles 3 H wave and M wave by electromyography. Results The range of initiative motion of ankle dorsiflexion and the sEMGMax of the anterior tibial muscle of two groups patients increased significantly after four weeks treatment (P 〈 0.01 ). The CSS assessment and HMax/MMax of the calf muscles 3 increased significantly only in the control group after treatment(P 〈0.01 ) ,but not in the treatment group. After treatment, compared to the control group, the range of initiative motion of ankle dorsiflexion and the sEMGMax of the anterior tibial muscle in the treatment group increased significantly(P 〈 0.01 ). After treatment, compared to the treatment group, the CSS assessment and HMax/MMax of the calf muscles 3 in the control group increased significantly ( P 〈 0.01 ). Conclusion Auto Move AM800 combined low-frequency electrical stimulation with the biofeedback skill can curefoot drop of cerebral infarction patients by improving the strength and spasm of muscle, and it also has a rebuild role to the damaged brain.
