反复扩缩式快速扩弓矫治器配合前方牵引矫治恒牙早期安氏Ⅲ类错的临床疗效观察

Correction of skeletal Class Ⅲ malocclusion by maxillary protraction combined with rapid repetitve expansion and constriction in early permanent dentition

目的观察上颌固定式快速扩弓矫治器配合面具前方牵引矫治恒牙早期以上颌骨发育不足为主的安氏Ⅲ类骨性错的临床疗效。方法选择17例恒牙早期以上颌骨发育不足为主的安氏Ⅲ类骨性错病例,采用附垫的上颌固定式快速扩弓矫治器配合面具前方牵引进行阻断矫治,并在矫治8或9周内对牙弓行反复扩缩,对前方牵引治疗前后X线头颅侧位片进行测量分析。结果 17例患者前牙反均解除,凹面型改善明显;X线头影测量分析显示SNA增加,A点前移明显,上前牙唇倾,上唇突度增加。结论附垫的上颌固定式快速扩弓矫治器配合面具前方牵引可有效治疗恒牙早期以上颌骨发育不足为主的安氏Ⅲ类骨性错。

Objective To evaluate the treatment effects of maxillary protraction combined with repectitive expansion and constriction on patients with skeletal Class Ⅲ malocclusion in early permanent dentition.Methods 17 skeletal Class Ⅲ patients with mainly maxillary retrusion in early permanent dentition were treated by maxillary protraction combined with rapid maxillary expansion and constriction.Cephalometric analysis was carried out before treatment and after maxillary protraction therapy and treatment.Results Better improvement of discrepancy of maxilla and mandible could be obtained in all the patients.Cephalometric analysis showed that SNA and ULP increased,point A protracted obviously,and the upper incisors tipped labially.After treatment,the overjet and molar relationship improved evidently.Conclusions Maxillary protraction with repetitive expansion and constriction are helpful and specific in treating skeletal Class Ⅲ patients with mainly maxillary retrusion in early permanent dentition.
