中西医结合治疗产后抑郁症临床疗效的Meta分析

Meta-analysis on clinical effects of integrated traditional Chinese and Western medicine for treating postpartum depression

目的:评价中西医结合治疗产后抑郁症的临床疗效。方法:全面检索外文数据库Pubmed(1979～2010)、ScienceDirect(1989～2010);中国生物医学文献数据库(CBM)、中国期刊全文数据库(CNKI)(1979～2010)、维普中文科技期刊全文数据库(VIP)(1989～2010)和万方医药期刊数据库(WF)(1989～2010),收集单用西药与中西医结合治疗产后抑郁症的随机对照临床实验,制定文献纳入及排除标准,由2名研究者分别独立筛选文献,符合纳入标准的文献按Juni量表进行质量评价。应用Review Manger4.2.2软件进行Meta分析,计算治疗组与对照组的总有效率的比数比(OR)及其95%CI。结果:符合纳入标准的随机对照实验有9篇文献,共719例患者。Meta分析结果显示,中西医结合治疗能有效治疗产后抑郁症[OR=4.83,95%CI为(2.75 8.51),P<0.01]。结论:Meta分析提示中西医结合治疗与单用西药治疗产后抑郁症比较,疗效差异有统计学意义,但确切疗效仍有待于临床开展大规模、多中心随机对照试验(RCT)研究来证实。

Objective：To evaluate the clinical effectiveness of combined use of traditional Chinese and Western medicine for treating postpartum depression.Methods：By searching the major databases of Pubmed,ScienceDirect,CBM,CNK,VIP and WF,we collected the data of randomized controlled trails pertaining to combined use of traditional Chinese and Western medicine in the treatment of postpartum depression.According to including and excluding criteria,the articles were evaluated.Randomized control trails（RCTs） were assessed according to Juni assessment.Software Review Manger 4.2.2 was used to assess the control trails.Results：Nine randomized controlled trails involving totally 719 patients were collected according to the inclusion criteria,and meta-analysis of the data indicated that combined use of traditional Chinese and Western medicine could be of great value in the management of postpartum depression in comparison with exclusive use of Western medicine [combined odds ratio（OR） was 4.83 with 95% confidence interval（CI）（2.75,8.51）,Z=5.64,P0.01].Conclusion：The clinical evidence derived from the analysis suggests that the combination of traditional Chinese and Western medicine for treating postpartum depression can be effective with good security.
