奥扎格雷对缺血性脑卒中患者颈动脉斑块和颈部血流的影响

The influence of Ozagrel for the carotid plaque and cervical flow of the patients with ischemic stroke

目的:探讨奥扎格雷对缺血性脑卒中患者颈动脉斑块和颈部血流的影响。方法:选取2009年1月～2010年12月于我院进行治疗的72例缺血性脑卒中患者为研究对象,将其随机分为对照组(常规治疗组)36例和观察组(加用奥扎格雷组)36例,将两组患者总有效率、颈动脉斑块及颈部血流进行检测及比较。结果:观察组总有效率(94.44%)高于对照组(80.56%)(χ2=5.367,P<0.05),颈动脉斑块总数[(1.95±0.29)个]少于对照组[(2.41±0.30)个](t=6.458,P<0.05),斑块大小[(12.02±2.09)mm]小于对照组[(18.97±2.31)mm](t=6.386,P<0.05),管腔狭窄比[(27.6±3.5)%]低于对照组[(44.6±3.7)%](t=5.983,P<0.05);颈总动脉近端和远端的收缩期峰值血流速度[(92.3±10.2)cm/s,(72.3±9.6)cm/s]、平均血流速度[(24.1±4.2)cm/s,(21.8±3.6)cm/s]、血管阻力指数[(0.80±0.19,(0.77±0.14)]均优于对照组的收缩期峰值血流速度[(86.1±10.3)cm/s、(68.2±9.4)cm/s]、平均血流速度[(20.6±3.8)cm/s、(17.3±3.5)cm/s]及血管阻力指数[(0.47±0.20)、(0.54±0.21)],差异均有统计学意义(t=5.121、5.862、5.779、5.689、6.635、7.148,均P<0.05)。结论:奥扎格雷对缺血性脑卒中患者颈动脉斑块和颈部血流的影响较明显,可显著改善血流循环及减小斑块。

Objective： To study the influence of Ozagrel for the carotid plaque and cervical flow of the patients with ischemic stroke. Methods： 72 patients with ischemic stroke in our hospital from January 2009 to December 2010 were selected as research object, and they were randomly divided into control group （routine treatment group with 36 cases） and observation group （ozagrel group with 36 cases）, then the total effective rate, carotid plaque and cervical flow of two groups were detected and compared. Results： The total effective rate of observation group （94.44%） was higher than that of control group （80.56%） （χ^2=5.367, P〈0.05）, number of carotid plaque [（1.95±0.29）] was less than that of control group [（2.41±0.30）,（t= 6.458, P〈0.05）, patch size [（12.02±2.09） mm] was smaller than that of control group[（18.97±2.31） mm] （t=6.386, P〈0.05）, lumen stenosis proportion [（27.6±3.5）%] was lower than that of control group[（44.6±3.7）%] （t=5.983, P〈0.05）. Proximal common and distal common carotid artery of PSV [（92.3±10.2）cm/s, （72.3±9.6） cm/s], VM [（24.1±4.2）cm/s, （21.8±3.6）cm/s] and BI [（0.80±0.19, （0.77±0.14）]were all better than PSV[（86.1 ± 10.3）cm/s, （68.2±9.4）cm/s], VM[（20.6±3.8）cm/s, （17.3 ±3.5）cm/s] and BI [（0.47±0.20）, （0.54±0.21）] of control group, there were all significant differences （t=5.121, 5.862, 5.779, 5.689, 6.635, 7.148, all P〈0.05）. Conclusion： The influence of ozagrel for the carotid plaque and cervical flow of the patients with ischemic stroke is obvious, and it can significantly improve the blood circulation and decrease carotid plaque.
