社区中老年男性前列腺体积相关参数和患病情况分析

Survey on parameters of prostate volume and prevalence of prostate diseases among middle-aged and elderly males in Hebei communities

目的:调查社区健康中老年男性的前列腺体积(PV)相关参数及患病情况,比较城镇与农村居民的前列腺参数和患病差异。方法:使用统一的结构式问卷调查1498例40～69岁健康男性,获得有效问卷1480份,使用国际前列腺症状评分(IPSS)评价下尿路症状(LUTS),其中434例测定或者计算血清生殖激素水平,650例使用经腹部B超测量PV相关参数。结果:3个年龄组PV的中位数分别为17.10ml、18.70ml和20.21ml,IPSS评分的中位数分别为1.64分、3.05分和2.07分,中重度LUTS患病率分别为10.91%、29.20%和22.72%,良性前列腺增生(BPH)平均患病率为13.85%。另外,城镇研究对象的PV、IPSS评分和BPH患病率均高于农村。结论:PV随着增龄呈现梯度性增大,IPSS评分和LUTS也存在随着增龄而增加的趋势。

Objective： To survey parameters of prostate volume （PV） and the prevalence diseases of prostate among middle - aged and elderly males living in the township and rural communities. Methods： A total of I 498 healthy males aged 40 to 69 years were investigated By an unified structured questionnaire, and 1 480 valid questionnaires were collected. Lower urinary tract symptoms （LUTS） were evaluated with international prostate symptom score （IPSS）. A total of 434 males were assigned to determine or calculate concentrations of serum reproductive hormones, and 650 to measure parameters of PV by transabdominal sonography. Results ： Of three groups with different ages, the median PVs were 17.10 ml, 18.70 ml and 20.21 ml, respectively. The median IPSSs were 1.64, 3.05 and 2.07, respectively. The prevalence rates of the moderate and severe LUTS were 10.91%, 29.20% and 22.72%, respectively. The average prevalence rate of Benign prostatic hyperplasia （BPH） was 13.85%. Furthermore, PV and IPSS as well as the prevalence rate of BPH of males living in township communities were higher than those of males living in rural communities. Conclusion： PV appears the gradient enlargement with age. And IPSS and the prevalence of LUTS also have a trend of increase with age.
