摘要
目的探讨重组人促红细胞生成素(recombinant human erythropoietin,rhEPO)对脑出血患者神经元再生的影响。方法将41例脑出血患者随机分为两组,对照组和治疗组均给予脑出血的常规处理,在此基础上治疗组每例患者给予rhEPO 5 000 U,经腰椎穿刺鞘内注入,分别于第1、4、7、10、14、17、21天给予1次。两组患者分别于鞘内注药的第1、4、7、10、14、17、21天,采集清晨空腹肘静脉血3 ml(生化管),室温放置待凝固后,以3 000 r/min离心10 min,分离血清,置于-20℃冰箱保存待测血管内皮细胞生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF),并采用欧洲卒中量表(ESS)评定患者的神经功能变化。结果治疗的第1、4天两组患者的血清VEGF、bFGF水平间差异均无统计学意义(P>0.05),而治疗的第7、10、14、17、21天两组患者的血清VEGF、bFGF水平间差异均有统计学意义(P<0.05)。治疗的第1、4、7天两组患者的ESS评分间差异无统计学意义(P>0.05),而治疗的第10、14、17、21天两组患者的ESS评分间差异均有统计学意义(P<0.05)。结论通过腰椎穿刺注入rhEPO能促进脑出血患者的神经元再生,改善神经功能。
Objective To explore the effectiveness of recombinant human erythropoietin(rhEPO) on neurons regeneration after intracerebral hemorrhage.Methods Fourty-one inpatients with hypertensive cerebral hemorrhage were randomly divided into two groups,both of which received routine treatment.Besides,patients in the treated group received rhEPO 5 000 U/d through lumbar intrathecal injection on day 1,4,7,10,14,17,and 21.Fasting peripheral venous blood of the two groups were obtained on the morning of day 1,4,7,10,14,17,and 21 for the purpose of VEGF and bFGF detection.Neural function changes of the patients were evaluated with Europe Stroke Scale(ESS).Results Differences of VEGF and bFGF between the two groups showed no statistical significance on day 1 and day 4(P0.05) and became statistically significant on day 7,10,14,17,and 21(P0.05).Differences of ESS between the two groups showed no statistical significance on day 1,4,and 7(P0.05) and became statistically significant on day 10,14,17,and 21(P0.05).Conclusion Lumbar intrathecal injection of recombinant human erythropoietin can promote neurons regeneration after intracerebral hemorrhage.
