前列腺癌患者外周血Matriptase和VEGF检测及其临床意义

摘要 目的联合检测前列腺癌患者外周血中Matriptase和血管内皮生长因子(vascular endothelial growth factor , VEGF)的表达、与前列腺癌临床参数的关系,探讨其临床意义。方法采用酶联免疫吸附ELISA法检测47例前列腺癌(其中20例远处转移)、45例良性前列腺疾病(20例前列腺炎、25例前列腺增生)、10例前列腺增生并高级乳头状瘤、20例正常前列腺者外周血Matriptase和VEGF的表达。结果前列腺癌患者血清Matriptase和VEGF的表达水平均高于良性前列腺疾病患者、PIN、及正常对照组,差异具有统计学意义;良性前列腺疾病、PIN、及正常对照组组间比较差异无统计学意义;二者在前列腺癌组织中的表达水平与其病理分级和临床分期均呈正相关,转移者明显高于原位癌,差异具有统计学意义;前列腺癌患者血清Matriptase和VEGF两者表达水平呈正相关;两者均高表达者转移迅速,预后差。结论Matriptase和VEGF与前列腺癌分期分级、恶性程度和预后密切相关,联合检测血清Matriptase和VEGF的表达可作为判断前列腺癌生物学行为及预后的重要指标。 Objectives To investigate the serum levels of Matriptase and vascular endothelial growth factor (VEGF) in patients with prostate diseases, and their relationship with the clinicopathological parameters, and evaluate its significance for diagnosing prostate cancer. Methods The serum levels of specimens of 122 cases ,27 localized prostate cancer,20 metastatic prostate cancer,20 chronic prostatitis (CP) ,25 benign prostatic hyperplasia( BPH), 10 high -grade intraepithelial neoplasias(PIN) ,and 20 NP were selected, were detected by ELISA. Results Serum levels of Matriptase and VEGF were significantly higher in patients with prostate cancer than those in BPH, CP, PIN, and NP( p 〈0.01 ), There no significant difference in BPH, PIN, CP, and NP groups;The positive correlation was noted between Matfiptase and VEGF expression, and both of them were significantly related to the tumor pathologic grades and tumor clinical stage( p 〈0.05 ). Serum concentrations of Matriptase and VEGF in patients with metastatic prostate cancer was significantly higher than that in localized prostate cancer( p 〈 0. 01 ). Conclusions Serum level of Matriptase and VEGF are closely related to the histopathological grading, the tumor differentiation and prognosis of Pca;The combined detection of Matriptase and VEGF could be used as parameters for assessing the biological behavior and prognosis of Pca.
