摘要
目的:探讨一种新型颈前路Uniplate钛板系统的初步临床应用效果及其设计特点和临床优势。方法:2008年8月-2009年3月,采用Uniplate钛板及碳纤维cage对11例患者进行颈前路单节段椎间盘切除、植骨融合术。其中Hangman骨折伴C2-3椎间盘损伤6例;颈椎间盘突出并颈椎不稳3例;急性颈椎间盘突出2例。评价及分析Uniplate钛板的新型设计、术中操作特点及患者术后恢复情况。结果:所有患者内固定安放顺利,手术时间平均90min,出血平均85ml。无喉返神经及食管牵拉损伤等相关并发症发生。术后X线片示内固定位置良好,颈椎生理曲度恢复。结论:Uniplate颈前路钛板系统的独特设计,使其术中操作更加便捷,显著降低手术难度,而且有效减轻软组织牵拉。其初步临床应用效果较好,但远期效果还需大宗病例长期随访观察。
Objective:To explore the preliminary clinical effect,design characteristic and clinical advantages of a novel anterior cervical Uniplate plate system.Method:From December 2008 to March 2009,11 patients were treated with anterior cervical discectomy fusion at a single segment,using Uniplate titanium plate for fixation and carbon fiber cage for bone grafting.Patients were diagnosed as Hangman’ fracture with C2-3 disc injury in 6 cases,cervical spondylotic myelopathy with cervical instability in 3 cases,and cervical spondylotic esophagopathy in 2 cases.The new design and operating features of the Uniplate titanium plate,as well as rehabilitation and postoperative radiographs of the patients were evaluated and analyzed.Results:All the patients were instrumented successfully.The operating time was at a mean of 90 minutes and the mean blooding volume was 85ml.No stretch injury of recurrent laryngeal nerve or esophagus occurred.The postoperative X-ray images demonstrated good placement of the implants and restoration of cervical curvature.Conclusion:The unique design makes Uniplate more simple and convenient to manipulate in operation,therefore decreasing the difficulty of surgical procedure dramatically and reducing the stretch of soft tissue effectively.Though it showed a good result in preliminary clinical application,its long-term effect should be identified by a long term follow-up to sufficient cases.
