改良Chiari氏截骨术治疗重度儿童股骨头缺血性坏死的护理

Nursing Care of Modified Chiaris’s Osteotomy in the Treatment for Severe Avascular Necrosis of the Femoral Head in Children

目的:探讨改良Chiari氏截骨术治疗重度儿童股骨头缺血性坏死的护理方法。方法:对15例改良Chiari氏截骨术患儿术前给予心理护理,术后给予观察伤口情况、监测体温变化、支具使用指导及功能锻炼。结果:所有患儿获得随访,随访6个月～24个月,髋关节功能明显改善,术后一年时伸屈可达0-120°,疼痛消失,跛行明显改善。复查X片示股骨头均覆盖良好,骨盆截骨处骨愈合,截骨线模糊的时间为6～12周,平均8周。结论:改良Chiari氏截骨术配合系统的康复指导对于解除髋部疼痛,恢复髋关节功能的疗效显著。

Objective：To discuss nursing methods of modified Chiaris’s osteotomy in the treatment for severe avascular necrosis of the femoral head in children.Methods：15 cases of modified Chiaris’s osteotomy in children were offered psychological nursing before the operation,observing the condition of the wound,taking temperature,guiding the use of the brace and functional exercise.Result：All cases were followed up from 6 to 24 months.Hip function was improved obviously,flex could reach 0-120°one year after operation,pain disappeared,and walking with a limp was improved obviously.X-ray films were shown the femoral head covered good;pelvic osteotomies were healed and osteotomies lines were fuzzy at 6 to 12 weeks postoperatively,the average were 8 weeks.Conclusion：The curative effect of modified Chiaris’s osteotomy coordination with systematic rehabilitation guidances for the hip pain relieving andthe hip fuction recovery is distinct.
