超声应变和应变率显像技术对肥厚型心肌病患者左室收缩功能的评价 被引量:4

摘要 应用超声应变及应变率显像技术评价肥厚型心肌病(HCM)患者左室局部心肌收缩功能。方法选取38例HCM患者作为观察组,选取健康志愿者36例作为对照组,分别进行组织多普勒超声检查,应用应变和应变率显像技术对局部心肌进行定量分析和评价。结果观察组患者左房内径、左室内径、左室收缩期末内径、室间隔厚度、左室后壁厚度均明显增加,左室射血分数明显减少(均P〈0.05)。观察组患者左室下壁、侧壁、后壁、前壁及室问隔心肌收缩期峰值应变、应变率均明显低于对照组(均P〈0.05)。结论应变和应变率显像技术能准确反映HCM患者左心室局部心肌收缩功能减退及特性的改变。 To evaluate regional myocardial systolic function in patients with hypertrophic eardio- mynpathy(HCM) by strain and strain rate imaging. Methods 38 patients with HCM( observe group) and 36 healthy volunteers (control group) were involved in this study. All subjects were received conventional 2D-Color Doppler echo- eardiography and Color Doppler myocardial image ( CDMI ), then analyzed the regional myocardial function by strain and strain rate imaging. Results Comparwed with the control group,left atrial diameters (LAD), left ventrleular di- ameters ( LVD), left ventricular end-systolic dimension ( LVDs ), interventrieular vemricular septal (IVS) and left ven- tricular posterior wall (LVPW) were significantly increased in patients of observe group ( all P 〈 0.05 ), but left ventrie- ular ejection fraction(LVEF) was decreased( P 〈 0.05 ). Patients with HCM showed regional longitudinal peak systol- ic myocardial deformation properties lower than those of counterparts at inferior, lateral, posterior, frontal and septum wall of left ventrieular( all P 〈 0.05 ). Conclusion Hypertrophic eardiomyopathy was associated with significant re- duetion in systolic function of left ventricle and strain and strain rate imaging was useful in evaluating the change.
