经睑缘切口微创中面部提升和下睑区域年轻化

Lifting of midface and rejuvenation of region palpebralis inferior through palpebral margin incision

目的探讨经睑缘切口微创中面部提升和下睑区域年轻化的方法和效果。方法常规行睑袋睑缘切口，离断眼轮匝肌支持韧带并松解颧前间隙，将眼轮匝肌上提、固定，以获得中面部提升和下睑区域年轻化的效果。自2010年6月至2011年3月，共治疗35例患者，平均年龄51岁（35～65岁）。并根据术前、术后照片或电话随访进行临床评价。患者满意度分为不满意、满意和非常满意。结果术后随访患者3～12个月，均未出现睑外翻和面神经损伤等并发症。其中，23例患者的睑颊沟外观得到明显改善。25例患者（占71．4％）对手术效果非常满意，10例患者（占28．6％）为满意。结论经睑缘切口离断眼轮匝肌支持韧带并松解颧前间隙的方法，切口小，无面神经损伤的风险，可有效提升中面部，使睑颊沟凹陷明显改善。

Objective To investigate the method and effect of lifting of midface and rejuvenation of region palpebralis inferior through palpebral margin incision. Methods Through palpebral margin incision, retinacular ligament of musculus orbicularis oculi was dissected, prezygomatic interspace was released and musculus orbicularis oculi was lifted and fixed to achieve the lifting of midface and rejuvenation of region palpebralis inferior. From June 2010 to March 2011, 35 patients averaged age 51 years old （35 to 65） underwent the operation mentioned above. Clinical evaluation was carried out preoperatively and postoperatively with standardized digital photographs or telephone follow-up. Degree of satisfaction was classified as non-satisfaction, satisfaction, and very satisfaction. Results The follow-up was performed for 3 to 12 months, and all cases were without ectropion and facial nerve injury. Of 35 patients, the eyelid sulcus in 23 patients were improved obviously. Very satisfied result was achieved in 25 patients （71.4%）. Ten patients were satisfied （28.6%）. Conclusion The method of dissecting retinacular ligament of musculus orbicularis oculi and releasing prezygomatic interspace through palpebral margin incision could lift midface effectively, improving depression of eyelid snlcus with slight incision and non-injury of facial nerves.
