Reference ranges of peripheral blood lymphocyte subset percentages in hepatitis B virus-infected Chinese population

目的调查中国乙型肝炎患者外周血淋巴细胞亚群频率参考值范围。方法利用流式细胞术检测2846例乙型肝炎患者和117例健康人群外周血淋巴细胞亚群数值,调查我国健康人群和乙型肝炎人群的参考值范围。结果调查了16～60岁健康人群和HBV感染相关的急性肝炎、慢性肝炎、重型肝炎和肝硬化人群外周血CD3+T淋巴细胞、CD3+CD4+T淋巴细胞、CD3+CD8+T淋巴细胞、CD4+/CD8+比值、CD3-CD19+B细胞、CD3-CD16+CD56+自然杀伤细胞(natural killercell,NK细胞)以及CD3+CD16+CD56+NKT细胞亚群频率参考值范围。结果显示,HBV感染不同疾病进展人群外周血CD4+T淋巴细胞频率与成年健康人群比较显著升高,而NK和NKT细胞频率显著降低。还发现外周血CD4+T淋巴细胞、CD4+/CD8+比值、NK以及NKT细胞亚群频率与年龄呈低度正相关,而CD8+T淋巴细胞和CD19+B细胞频率则与年龄呈低度负相关。结论通过长期大样本调查,提出我国HBV感染的成年患者外周血淋巴细胞亚群频率参考值范围,为临床判断慢性乙型肝炎患者免疫状态提供参考依据。

Objective To investigate region-specific reference ranges of peripheral blood lymphocyte subset percentages in hepatitis B virus （HBV）-infected Chinese population. Methods Flow cytometry was used to detect the percentages of peripheral blood lymphocyte subsets in 2846 HBV-infected subjects and 117 healthy subjects in 302 Hospital of PLA from 2001 to 2009. The reference ranges of peripheral blood lymphocyte subset percentages in healthy and HBV-infected Chinese popula-tion were investigated. Results The reference ranges of the percentages of peripheral blood lymphocyte subsets including CD3＋ T lymphocytes, CD3＋CD4＋ T lymphocytes, CD3＋CD8＋ T lymphocytes, CD4＋/CD8＋ ratio, CD3-CD19＋ B cells, CD3-CD16＋CD56＋ natural killer （NK） cells and CD3＋ CD16＋CD56＋ NKT cells were defined in healthy adults and HBV-infected patients with acute hepatitis B, chronic hepatitis B （CHB）, liver failure and liver cirrhosis （aged from 16 to 60 years old）. It was found that the percentages of peripheral CD4＋ T lymphocytes significandy increased, and NK cells and NKT cells significantly decreased in HBV-infected adult patients as compared with healthy adults. Further correlation analysis indicated that the age of all the subjects was slightly positively associated with the percentages of peripheral CD4＋ T lymphocytes, CD4/CD8 ratio, NK cells and NKT cells, but slighdy negatively associated with CD8＋ T lymphocytes and CD19＋ B ceils. Conclusions Through long-term and large-sample investigation, the region-specific reference ranges of peripheral blood lymphocyte subset percentages in HBV-infected adult Chinese patients are proposed. They will be helpful for the evaluation of immune status of the HBV-infected subjects in clinic.
