摘要
目的:探讨阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apnea hyponea syndrome,OSAHS)及OSAHS合并高血压(obstructive sleep apnea hyponea associated hypertension,OSAHAHT)患者血清白细胞介素-6(interleukin-6,IL-6)、血管内皮生长因子(vascular endothelial growth factor,VEGF)水平的变化及其临床意义。方法:选健康对照组30例、OSAHS30例,OSAHAST24例,用酶联免疫法(ELISA)测定血清VEGF、IL-6含量。所有患者采用多导睡眠图(polysomnography,PSG)监测。结果:(1)睡眠呼吸紊乱指数(apnea hypopnea index,AHI)、脉搏氧饱和度(SpO2)<90%时间占总睡眠时间的百分比(TST-SP<90%)OSAHAHT组较OSAHS组显著升高(P<0.01);平均血氧饱和度(MaO2)、最低血氧饱和度(LaO2)OSA-HAHT组较OSAHS组显著降低(P<0.05);2组间睡眠呼吸暂停最长时间(LAT)无显著差异;(2)血清IL-6、VEGF水平OSAHAHT组显著高于OSAHS组(P<0.01),2组与正常对照组均有显著差异(P<0.01);(3)血清IL-6、VEGF浓度均与AHI呈正相关,血清IL-6浓度还与TST-SP<90%呈正相关,血清VEGF浓度与MaO2呈负相关;(4)血清VEGF与IL-6水平呈正相关。结论:VEGF、IL-6可能共同参与了OSAHS的病理生理过程并促进高血压的发生、发展。
Objective:To investigate the role of interleukin-6(IL-6),vascular endothelial growth factor(VEGF) in the pathogenesis of obstructive sleep apnea hyponea syndrome(OSAHS)and obstructive sleep apnea hyponea associated hypertension(OSAHAHT).Methods:A total of 30 OSAHS cases,24 OSAHAHT cases and 30 healthy control cases were enrolled.The serum levels of IL-6 and VEGF were measured.Polysomnography(PSG) were used in all participants.Results:(1) Apnea hypopnea index(AHI) and TST-SP90% of patients with OSAHAHT were significantly higher than those with OSAHS,but MaO2、LaO2 were significantly lower.There were no significant difference between the two groups′LAT.(2) The serum levels of IL-6 and VEGF in patients with OSAHAHT were significantly higher than those of OSAHS cases.Compared with healthy control group,both the two groups had significant difference.(3) Both of the serum IL-6 and VEGF level were positively correlated with AHI.There was positive correlation between the serum IL-6 level and TST-SP90%,negative correlation between VEGF and MaO2.(4) There was positive correlation between the serum IL-6 and VEGF level.Conclusions: IL-6 and VEGF probably took part in the progress of OSAHS commonly and accelerated the occurrence and development of hypertension.
