窒息早产儿脑血流和脑电图的改变及临床意义 被引量:2

摘要 目的探讨窒息早产儿脑血流动力学和脑电图的改变及与窒息程度的关系。方法采用TCD、常规脑电图检测43例早产的窒息患儿,20例早产无窒息的新生儿脑血流动力学及脑电图的改变。结果轻度窒息组的Vs、Vd及Vm均高于对照组,重度窒息组Vs、Vd及Vm明显高于对照组,并高于轻度窒息组,有显著性差异。轻度窒息组RI低于对照组,而重度窒息组RI明显减低,与对照组及轻度窒息组相比均有显著性差异。20例对照组中,脑电图轻度异常1例,异常率5%;未见中度及重度异常者;轻度窒息组15例,EEG异常5例,占总窒息组的11.63%;重度窒息组28例,全部异常,其中重度异常2例(随访中2例死亡,另1例出现脑瘫),中度异常16例,轻度异常10例(组间比较有显著性差异,P<0.05)。结论早产儿脑血流动力学和脑电图的变化可作为判断窒息程度和临床预后的可靠指标。
