头孢硫脒氨溴索治疗小儿社区获得性肺炎158例疗效观察

Observation of the Therapeautic Effect of Cephathiamidine Combined with Ambroxol Treatment Community-acquired Pneumonia 158 cases

目的观察头孢硫脒氨溴索治疗小儿社区获得性肺炎(CAP)的疗效。方法将314例CAP患儿随机分为治疗组(158例)和对照组(156例),两组患儿均给予常规保持呼吸道通畅、吸痰、吸氧、雾化等对症、支持治疗。在此基础上,治疗组给予头孢硫脒50 mg/kg,同时给予氨溴索7.5 mg/kg。对照组在常规治疗基础上给予头孢唑啉注射剂50 mg/kg。观察两组患儿用药期间发热、咳嗽、气促、肺部细湿啰音消失、胸部X线炎症吸收等情况。结果治疗组总有效率为94.9%,对照组总有效率为61.6%,两组比较差异有统计学意义(P<0.05)。治疗组临床症状明显改善,与对照组比较差异有统计学意义(P<0.05)。两组患者中金黄色葡萄球菌、肺炎球菌、大肠杆菌感染时,治疗组细菌阴转率明显高于对照组,两组比较差异有统计学意义(P<0.05)。结论头孢硫脒和氨溴索两者联用治疗肺炎,相互促进疗效较好,是一种治疗CAP的有效方法,值得推广应用。

Objective To observe the therapeutic effect of cephathiamidine combined with ambroxol treatment community-acquired pneumonia（CAP） of children.Methods 314 CAP cases were randomly divided into treatment group（158 cases） and control group（156 cases）,the two groups were both given supporting treatments such as keeping airway open,sucking phlegm,oxygen and atomization etc.,based on which,the treatment group was given cefathiamidine 50 mg/kg,and ambroxol 7.5 mg/kg at the same time.And for the control group based on conventional treatment,cefazolin injection 50 mg/kg was given.Fever,cough,shortness of breath,pulmonary fine wet rales disappearance,chest x-ray inflammation absorption etc.of the children in two groups during the administration were observed.Results The total effective rate of the treatment group was 94.9%,the total effective rate of the control group was 61.6%,the difference was statistically significant（P〈0.05）.The clinical symptoms of the treatment group were obviously improved with significant difference to the control group（P〈0.05）.Bacterial negative conversion rate in treatment group Staphylococcus aureus,escherichia coli,pneumococcal infections,was obviously higher than the control group,there was significant difference between two groups（P〈0.05）.Conclusion Cefathiamidine and ambroxol combined treatment for pneumonia,has good synergy effect,making it an effective treatment for CAP,worthy of popularization.
