单肺通气不同的通气方式对呼吸和循环的影响 被引量:2

摘要 目的探讨开胸手术肺隔离时行单肺通气(OLV),不同通气方式对病人术中呼吸和循环的影响。方法择期开胸手术40例,ASA I-II。随机分小潮气量加呼气末正压(PEEP4-6mmHg)A组(n=20),正常潮气量B组(n=20)。分别于麻醉后双肺通气时T0,单肺通气后20minT1,单肺通气后40minT2。单肺通气后60minT3。记录气道峰压Peak、气道平台压Plat、动脉血氧分压PaO2、二氧化碳分压PaCO2、血氧饱和度SpO2、心率HR、血压BP、中心静脉压CVP。结果两组病人单肺通气后氧分压较双肺通气时明显降低,A组T1与B组比SpaO2、PaO2、有统计学差异(P<0.05)。A组加PEEP后SpaO2、PaO2、升高,T2、T3与B组比较无统计差异(P>0.05)。两组循环稳定,B组气道压明显高于A组(P<0.05)。结论单肺通气采用小潮气加合适的PEEP模式可以增加血氧饱和度,提高氧分压,对气道压、循环无影响。
