老年慢性阻塞性肺疾病急性加重期患者检测hs-CRP和前白蛋白的临床意义 被引量:3

摘要 目的观察高敏C反应蛋白(hs-CRP)和前白蛋白(PA)在慢性阻塞性肺疾病急性加重期(AECOPD)患者治疗前后的变化,分析其在AECOPD病情监测及疗效观察中的应用价值。方法检测115例老年AECOPD患者血清PA、hs-CRP、血沉(ESR)和白细胞(WBC)含量,与55例COPD稳定期患者进行比较;AECOPD患者根据预后分成死亡组和存活组,比较2组入院时和治疗7 d后PA和hs-CRP的差异。结果 AECOPD患者治疗前血清PA、hs-CRP与COPD稳定患者相比有显著性差异,WBC和ESR无显著性差异。死亡组入院第1天hs-CRP、PA与存活组比较无明显差异;死亡组第7天PA较第1天降低,而hs-CRP与第1天相比无明显变化。存活组第7天hs-CRP较第1天降低,PA较第1天升高。第7天存活组hs-CRP水平明显低于死亡组,PA水平明显高于死亡组。结论 PA、hs-CRP参与了AECOPD的发病过程,动态监测血清PA、hs-CRP对于评价AECOPD患者病情具有一定的临床意义。
