腹腔镜复杂胆囊切除术的应用体会

Study of complex laparoscopic cholecystectomy

目的:总结复杂胆囊行腹腔镜手术的临床经验。方法:回顾分析2007年8月至2009年8月为54例患者行腹腔镜复杂胆囊切除术的临床资料。结果:52例顺利完成手术,2例中转开腹,其中1例因有上腹部手术史,难以建立气腹,1例因胆囊床出血镜下难以止血。结论:随着腹腔镜医师技术水平的提高,许多复杂胆囊也可行微创手术治疗。

Objective： To study the clinical experience of difficult laparoscopie eholeeystectomy. Methods： A retrospective analysis was made on clinical data of 54 cases of difficult laparoscopic choleeystectomy fi＇om Aug. 2007 to Aug. 2009. Results ： Among the 54 patients,LC was completed in 52 cases;2 eases were converted to open surgery. The conversion reasons included difficult creation of pneumoperitoneum caused by upper abdominal operation history, and hemorrhage in gallbladder fossa which was hard to stop. Conclusions ： With the development of laparoscopic skill,it is safe to perform laparoscopic mini-invasive treatment for complicated gallbladder.
