急诊床边应用支气管镜治疗重症Ⅱ型呼衰(附62例报告)

目的了解床边应用支气管镜治疗重症Ⅱ型呼衰的疗效,探讨床边急诊治疗的应用价值。方法床边经鼻或经口插入支气管镜予吸引分泌物、通畅气道、同时吸氧等综合治疗,必要时经支气管镜治疗后行气管插管,或机械通气。结果 62例重症Ⅱ型呼衰病人经床边支气管镜吸引治疗后22例意识当时就转清醒,经后期气管插管等综合治疗59例治愈好转,治愈好转率95.2%(59/62例)。结论床边应用支气管镜治疗重症Ⅱ型呼衰是一项有效的急救措施,对恢复气道通畅、清除气道分泌物、改善通气有明显效果。

Objective To understand the application of bedside bronchoscope on typeⅡrespiratory failure, and to evaluate the efficacy of bedside treatment in emergency. Methods Bedside bronchoscope through nasal or oral removes the secretions, unobstructes airway, supplies oxygen and other comprehensive treatment. If necessary, tracheal intubation or mechanical ventilation can be done after bronchoscopic treatment. Results 62 patients with severe typeⅡrespiratory failure patients were treated by the bedside bronchoscope, 22 cases had instance improvement, and 59 cases cured with the comprehensive treatment of post-intubation, the cure rate was 95.2% （59/62 cases）. Conclusion For severe typeⅡrespiratory failure patient, the bedside application of bronchoscope is an effective emergency treatment to restore the airway, clear airway secretions and improve the ventilation.
